Collect information from clients with custom forms — intake questionnaires, contact forms, feedback surveys, and more.

Creating a Form

Go to Website > Forms and click Create Form. Add fields: text, email, phone, dropdown, checkbox, date picker, file upload, and more. Drag to reorder.

Conditional Logic

Show or hide fields based on previous answers. For example, if they select "Wedding" as the event type, show additional fields for venue, guest count, and timeline.

Embedding

Embed forms on your website pages, share as a direct link, or include in emails. Submissions appear in your Forms dashboard with the client linked automatically.

Start from templates: Contact Form, Wedding Questionnaire, Portrait Intake, Event Details, Feedback Survey.
